Sella won a two-man sprint finish with Fortunato Baliani of Team Nippo.
Danilo Di Luca of Acqua e Sapone was third, two minutes and 50 seconds behind.
Only 10 non-Italians have won the semi-classic day race since its creation after World War Two.
On Friday there was no exception, with the top 10 riders all Italian.
It was the 31-year-old Sella’s biggest career win, with his three stages from the 2008 Giro d’Italia rubbed out after he was suspended for doping.
